For providers looking to refer a patient, here are some common identifiers for patients that need to be seen by a nephrologist.
- ANY patient with kidney disease
- Progression of CKD
- ANY patient with a GFR < 60 m/min
- ANY patient with an auto-immune disease
- Blood in the urine
- Difficult to treat edema
- Persistent electrolyte abnormalities DESPITE intervention
- Acid-Base disturbances requiring assistance
- Recurrent kidney stones
- Proteinuria/Albuminuria independent of ANY eGFR
- Acute Kidney Injury
- Albumin/Creatinine Ratio > 30 mg/g
- Vasculitis with renal manifestations
- Hereditary kidney disease
- Hypertension refractory to treatment with 4+ antihypertensive agents
- Urinary red cells casts not readily explained
